The Community Health Needs Assessment (CHNA) Committee (Bronson Healthcare, Oaklawn, CareWell Services Southwest, the Calhoun County Public Health Department, The Coordinating Council and Population Health Alliance) is pleased to announce the launch of the 2024 Calhoun County Community Health Needs Assessment Survey. The CHNA Committee is dedicated to improving the health and well-being of our community through advocacy, education, and comprehensive health services. They strive to create a healthier environment for all residents by addressing health disparities and promoting equitable access to care.

The CHNA Survey is designed to collect comprehensive data on the health status, needs, and concerns of community members. By participating in this survey, residents will have the opportunity to voice their opinions and experiences regarding various health issues, including access to healthcare, mental health services, chronic disease management, and preventive care.

Key Objectives

1. Identify Health Priorities: Understand the most critical health issues affecting our community.
2. Improve Healthcare Services: Gather feedback to enhance the quality and accessibility of local healthcare services.
3. Promote Community Wellness: Develop targeted programs and initiatives to address identified health needs.
4. Engage Residents: Foster a collaborative approach to community health by involving residents in the decision-making process.
 

How to Participate

Residents can participate in the survey in several convenient ways:

  • · Online: Visit MiCalhoun.org to complete the survey online.
  • · In-Person: Surveys will be available at local community centers, libraries, healthcare facilities and at events.
  • · Social Media: CHNA Committee members will be posting the survey on their social media pages.

The survey will be open from May 29, 2024 to August 16, 2024. All responses are anonymous and confidential and will be used solely for the purpose of improving community health services.
